Pre-Consultation Questionnaire

The first step in getting your pet's health certificate is to fill out this form and email it back to me. Please fill it out as accurately as possible as the names and addresses will be copied directly onto your certificate. It is fine to leave sections blank if you don't have concrete travel plans yet.

This is a PDF form that you can edit directly on your phone or computer, save, and then email to me at StLTravelVet@outlook.com. 

Alternatively, you can print the form and write your responses in and then scan or take pictures to email back to me. 

***To edit the form on an iphone***

1. Download the form
2. Click the button at the bottom of your phone's screen that looks like a box with an up arrow
3. Select the option to "markup" the document
4. Click the button at the top of the screen that looks like a pen and three dots. This will allow you to type your responses.
5. Save the document as a new file
